6th, 7th & 8th Grade Students: Please ask your PIE teacher for the permission slip to be able to ride on the Monster Bash Hayride this Friday, October 26. You will not be allowed on the hayride without a signed permission slip from your parents.
Box Top Challenge
Today is the deadline for the Box top challenge! Thank you ALL for earning money for our school!!! Phoenix middle school rocks because of YOU!!
Lost & Found
The Lost & Found has been spread out on the tables that are up against the wall in the cafeteria. Please go through and see if anything there is yours, if so take it with you. At the end of the month all items will be donated to charity. Parents! You are more than welcome to come and check out the Lost & Found Table before we donate everything if your child is missing something! It’s amazing how many lost & found items go unclaimed here at Phoenix Middle School!

Laser Tag
Students! There will be a Mother/Son Laser Tag Battle on November 9th at DDHS. Stop by the office for a flyer with the event info.
Seal-A-Smile
*ATTENTION 5th GRADE STUDENTS*
Please remember to return your completed SEAL-A-SMILE Dental Sealant permission slip By FRIDAY OCT. 26, 2018. The homeroom with the most completed returns will receive a color changing tumbler!!
Forms will be accepted until the day the dental assistants come in early November!! Remember to tell your parents/guardians that this is a FREE program!
Cool Weather Clothing
As the weather continues to cool down, be sure to bring your jacket, hat, gloves and anything else you may need with you to your lunch period. When lunch is done, you are NOT allowed to go back to your lockers to get clothing items for outside. Bring those items to the lunchroom when lunch begins.
Delavan Darien Youth Basketball

Tardy Consequences
Students, please review the tardy policy. There was a mistake in the previous information. On the 8th Tardy, you are required to serve In School Suspension.
1st Tardy: Nothing
2nd Tardy: Email from Mrs. Zahn to the student
3rd Tardy: 30 Minute Detention AFTER SCHOOL
4th Tardy: 30 Minute Detention AFTER SCHOOL
5th Tardy: 45 Minute Detention AFTER SCHOOL
6th Tardy: 45 Minute Detention AFTER SCHOOL
7th Tardy: 90 Minute Detention AFTER SCHOOL
8th Tardy: In School Suspension
9th Tardy: Citation (Ticket) from the Police Department
Cell Phone & Earbud Rules
Cell phones CAN NOT be used during your lunch time and/or during your recess time. Cell phones should be out of sight from 7:20 AM to 2:46 PM and earbuds/headphones need to be removed upon building entry. If you need to contact your parent during the school day, you can use the student phone in the front office. Please ask your teacher before coming down to the office and be sure to get a pass from your teacher.
